Print your forms or pick up at any office to complete & return!
All forms must be completed and documents provided, April 9th to the elementary office. Open office hours for enrollment will be from 9-11am or 5-7pm.
Due to lack of adequate space, parents and guardians will need to complete all forms BEFORE April 9th. Enrollment forms can be printed from the link (below) or picked up from any office.
*ALL students must provide TWO documents to prove residency. A current utility bill (within last 30 days) & Mortgage or Rental Agreement. Documents must identify the parent/guardian name, dated within the last 30 days and contain the service address.
Cut-off notices will NOT be accepted.
List of approved documents:
● Electric bill
● Gas bill
● Water bill
● Trash bill
● Mortgage Statement
● Housing Rental/Lease Agreement
The following documents are NOT acceptable for proof of residency. Including but not limited to:
● Internet bills (Pioneer, OEC Fiber)
● Drivers License
● Social Security cards/documents
● Cell phone bill
● Access card
